Porta John Consent Form

MAZZER PORTA JOHN DAMAGE WAIVER TERMS AND CONDITIONS / CONSENT FORM: Customer acknowledges that prior to taking the rented item(s), you examined it, saw it in operation (if appropriate), and are aware of its condition and that it is in good condition except for any defect noted on this contract. It is your responsibility to return the rented item(s) to Mazzer Porta John & Vac Services in the same condition. No infectious or hazardous waste shall be placed in the portable toilet. Customer acknowledges that he/she has care, custody, and control of equipment owned by Mazzer Porta John & Vac Services and accepts responsibility and liabilities for the use of the equipment and its contents. THEREFORE, THE CUSTOMER EXPRESSLY AGREES TO DEFEND, INDEMNIFY, AND HOLD HARMLESS Mazzer Porta John & Vac Services FROM AND AGAINST ANY AND ALL CLAIMS FOR LOSS OF OR DAMAGE TO PROPERTY OR INJURY TO OR DEATH OF PERSON OR PERSONS RESULTING FROM OR ARISING IN ANY MANNER OUT OF CUSTOMER'S USE OF THE EQUIPMENT FURNISHED UNDER THIS AGREEMENT. Customer accepts full responsibility for all damages to Mazzer Porta John & Vac Services property due to fire, theft, or vandalism, and agrees to pay reasonable costs of repair and/or replacement of the equipment or its components. Customer is responsible for theft or major structural damage to rental equipment owned by Mazzer Porta John & Vac Services, with the exception of a $250 deductible for a standard unit and $550 deductible for a wheelchair accessible unit. Mazzer Porta John & Vac Services will not be responsible for damage done to roadways/driveway surfaces, culverts, grass, etc., due to its equipment traveling on customer property, nor will Mazzer Porta John & Vac Services be responsible for damage done to roads, road surfaces, culverts, etc., on property or easements leading to or from the location at which the service is performed. Cancellation: I fully understand that I must cancel my porta potty rental by the 15th of the month prior to the next billing cycle (1st of the month). I understand that no refunds or pro rated fees will apply to a current month if I decide to terminate my rental early. This can be done by email or phone.

WAIVER PROTECTION OF RENTAL: Many customers do take advantage of the Damage Waiver protection to minimize risk of paying it of pocket for damages. This coverage protects you 24/7 while the unit is being rented. The damage waiver is a small price to pay at $20.00/month plus tax (per unit) to eliminate the risk of damage or vandalism. Damage Waiver agreement is not mandatory but strongly suggested. WAIVER PROTECTION OF RENTAL INCLUDES: Vandalism, Weather related damages, Minor damages.
