MAZZER PORTA POTTY CREDIT CARD & BILLING FORM

*By initialing below you are agreeing to our terms of service.

  • I fully understand that I must cancel my porta potty rental by the 15th of the month prior to the next billing cycle (1st of the month). I understand that no refunds or pro rated fees will apply to a current month if I decide to terminate my rental early. This can be done by email or phone.  
     

  •   Customer acknowledges that their card will not be charged until 24 hours before the initial drop-off date, and will NOT be charged prior to 24 hours before the initial drop-off date.
     

  • Customer acknowledges that prior to taking the rented item(s), you examined it, saw it in operation (if appropriate), and are aware of its condition and that it is in good condition except for any defect noted on this contract. It is your responsibility to return the rented item(s) to Mazzer Plumbing in the same condition. 
     

  • Customer acknowledges that he/she has care, custody and control of equipment owned by Mazzer Plumbing and accepts responsibility and liabilities for the use of the equipment and its contents. THEREFORE, THE CUSTOMER EXPRESSLY AGREES TO DEFEND, INDEMNIFY AND HOLD HARMLESS MAZZER PLUMBING FROM AND AGAINST ANY AND ALL CLAIMS FOR LOSS OF OR DAMAGE TO PROPERTY OR INJURY TO OR DEATH OF PERSON OR PERSONS RESULTING FROM OR ARISING IN ANY MANNER OUT OF CUSTOMER'S USE OF THE EQUIPMENT FURNISHED UNDER THIS AGREEMENT.
     

  • Customer accepts full responsibility for all damages to Mazzer Plumbing’s property due to fire, theft or vandalism and agrees to pay reasonable costs of repair and/or replacement of the equipment or its components.
     

  • Customer is responsible for theft or major structural damage to rental equipment owned by Mazzer Plumbing, with the exception of a $250 deductible for a standard unit and $550 deductible for a wheelchair accessible unit.
     

  • Mazzer Plumbing will not be responsible for damage done to roadways/driveway surfaces, culverts, etc., due to its equipment traveling on customer property, nor will Mazzer plumbing be responsible for damage done to roads, road surfaces, culverts, etc., on property or easements leading to or from the location at which the service is performed.
     

  • Any cancellation made within 24 hours of your rental/drop-off date will be charge a $50.00 cancellation fee to the credit card provided.
